ROUTE AND ACTIONS OF THE PATIENT FOR PLANNED HOSPITALIZATION

1. Hospitalization of patients with endocrine pathology in a hospital can be performed according to the decision of the hospitalization commission or heads of departments, the chief medical officer, his deputy for a medical part.

2. The direction on hospitalization is registered at the secretary of the chief medical officer.

3. In reception of a hospital to the patient the medical history with which he/she goes to stationary departments, where looks round the head and the attending doctor is made out.

Hospitalization in clinic is performed on the basis of the following documents:

1. The direction in a hospital;

2. It is desirable to have ordering from the out-patient card of policlinic or a medical history at the place of residence with the indication of the anamnesis of a disease, data of inspection (surely fluorography), treatment methods;

1. The passport (the birth certificate for children);

2. For children - obligatory certificates of carrying out preventive inoculations, certificates of a condition of a sanitary and epidemic environment.

3. Informed agreement of the patient (for children - the agreement of parents) about conditions of hospitalization, stay and implementation of regulations in a hospital.
